Kings Park Central School District

Kings Park Central School District’s Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students.  The Athletic Department, Coaches, Nurses and School Adminsitration will use this platform to verify athletic participation clearance.  Every student in KPHS and WTRMS interested in trying out for a sport starting with the upcoming Fall 2024 must create a FinalForms account, review and electronically sign all required forms and receive full clearance to be cleared to play a sport.  

The platform will become available for registration on Thursday, August 1st for Fall HS Sports and Saturday, August 10th for Fall WTRMS Sports.
